About The Shoulder Drug and Alcohol Recovery

The Shoulder is a Christian, nonprofit, treatment center meeting theneeds of those suffering from substance abuse problems.

TheShoulder offers outpatient services for both men and women, andresidential services for men.

They are now have offices in both Spanish Fortand Foley, Alabama.

The following substance abuse treatment services are available:

MEN AND WOMEN

  1. Assessments for Substance Abuse
  2. Outpatient substance abuse treatment
  3. Individual treatment
  4. Outpatient codependency group for family and friends affected by someone else's addiction. (No assessment needed)

MEN ONLY

  1. Residential substance abuse treatment          

Clients live onsite, work during the day, and receive treatment in the evenings

Daphne officeReferrals are also available for Department of Transportation evaluations, Suboxone treatment, and mental health counseling.

The Shoulder requires an assessment prior to admission into any of our treatment services.    

Those who are local to Baldwin County and Mobile may schedule an assessment at one ofThe Shoulder's two offices.

An assessment requires a non-refundable fee of $50.    

Those who are not local to the Mobile area should contact the mental health department in theircounty, and schedule a substance abuse assessment.

Ask the facility to send thecompleted assessment to The Shoulder by FAX to determine whether we are the rightplacement for treatment.

ONLY potential clients who are incarcerated, or who live in a county in which there is no mental health facility available, may download the following screening material.

If neither of these exceptions apply, the paperwork will not be accepted for potential admission.

Other requirements for potential clients include:

  • Outpatient and Residential programs   
  • A minimum age of 18   
  • A driver's license or valid picture identification card   
  • A social security cardResidential program only   
  • A work-ready physical from a doctor   
  • A current (within the last 6 months) TB test   
  • Proof of registration and automobile insurance if clients intend to operate a vehicle while in the residential program. 
  • A counselor must approve a clients privilege to operate a vehicle.    
  • A residential client must obtain full-time (40 hour per week) employment.
  • If a client does not have a job, he has 10 business days after admission to obtain gainful employment.

Federal Poverty Guidelines for 2023

This table is to be used if this rehab center accepts sliding scale treatment. This will show you what levels of income you need to make to qualify for free treatment.

For Households with more than 8 persons, add $4,480 for each additional person.

*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get legal aid from some offices, they use a sliding fee scale. When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
