About Centra Wellness Network

This is a sliding scale approved program

Centra Wellness Network provides substance abuse treatment services for adult men and women in an outpatient setting. These services include but are not limited to individual and group counseling, behavioral therapy, trauma related counseling, motivational interviewing, relapse prevention, case management, treatment for gambling disorder and anger management. Payment forms accepted include cash or self payment, medicare, state financed health insurance plan other than medicaid and medicaid. A sliding fee scale is available based off of income and other factors. Please contact Centra Wellness Network for more information on their fees and services.

For Households with more than 8 persons, add $4,480 for each additional person.

*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get legal aid from some offices, they use a sliding fee scale. When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
